You can combine Items when all the four slots are filled. All the bonuses associated with these new Items will go straight to the Hero's metrics, and you will be able to use the new Items right away.

Once you've combined a few Items, the level of other Items goes up. You'll have to spend some Silver on combining items, but it's well worth it, really! The specific costs depend on the level of the Item in question.
